The plant is native to isolated regions in Utah and Arizona, USA. Its distribution is restricted to "alcoves" or damp canyon walls where groundwater seeps provide a constant, cool, and moist environment. The specific epithet zothecina originates from the Greek word for a small room or alcove, accurately describing its extremely limited ecological niche.
Botanically, Platanthera zothecina displays a distinct morphology: it typically bears a single large basal leaf and a raceme of greenish-yellow flowers.
